*Bradley-Reid
Corporation: Provides case management services to people living
with HIV in Mecklenburg, Gaston, Cabarrus and Union Counties. Located

CAP/AIDS:
A program in Mecklenburg County providing in home care to people living
with AIDS. Call ATS Health Services (a private program), 704-342-2710
or the Health Department, 704-336-4674.
*Catawba
Care Coalition: Provides case management, support groups, counseling,
and medication adherence for Chester, Lancaster and York Counties. Linked
with the Christopher Clinic in Rock Hill, SC. 803-909-6363.
Clinics
providing treatment for Infectious Disease: Focus on the medications
and medical needs of HIV positive people. There are a several clinics
that exist and focus predominantly on HIV/AIDS. Physicians in family
practice or internal medicine may treat people with HIV or make a referral
to an HIV specialist.

*G.O.R.E.:
Provides case management for people living with HIV/AIDS. Located at
2118 Breezewood Drive, Charlotte, NC. 704-549-1953.
Hope Haven:
Is a residential and outpatient unit for people with substance abuse
issues. Located 3815 North Tryon Street, Charlotte, NC. 704-372-8809.
Hospice:
Provides services to clients in the last six months of life. These services
include case management, counseling, chaplain and nursing support. In
Charlotte, 704-375-0100.
House of
Mercy: Is a residential unit, primarily a hospice unit, for
people with advanced HIV disease. A diagnosis of AIDS is necessary to
gain admission. Located in Belmont; supervisor Shirley Stowe, 704-825-8832.
* The Jemsek
Project: Provides medical case management and support services
in the Charlotte metropolitan area. Located in Huntersville. For more

*Metrolina
AIDS Project: Provides client services for anyone with HIV
diagnosis including benefits advocacy, case management, counseling and
support groups in the Charlotte/Mecklenburg region. Located at 127 Scaleybark
Rd., Charlotte. 704-333-1435.
*Mecklenburg
Health Department: Testing for HIV/AIDS. Provides a variety
of services for people living with HIV including case management, CAP/AIDS
program and medication adherence counseling. Two locations in Charlotte:
2845 Beatties Ford Road (704-336-6500) and 249 Billingsly Road (704-336-5384).
SAIL:
Substance abuse treatment program including services for Latino population.

*Set Apart:
Provides case management for people living with HIV/AIDS in Mecklenburg,
Cabarrus, Gaston and Union Counties. 2121 Commonwealth Rd., Charlotte,

The Havens:
Residential unit for people living with HIV/AIDS and their families.
Approximately 25 units located on Creighton Avenue in Charlotte. 704-608-1436.
The Regional
HIV/AIDS Consortium: Provides funding for a variety of HIV/AIDS
related projects and services. Also provides supervision for case managers
in the county system and conducts the Workplace Alliance, education
on HIV/AIDS in the workplace. Located 7510 E. Independence Blvd., Charlotte,

*Total Care:
Provides case management for HIV positive clients who have Medicaid
in the Charlotte/Mecklenburg region. Located at 4530 Park Road, Charlotte,

United Family
Services: Provides counseling. Located 601 E. 5th Street, Charlotte,
NC. 704-332-9034.
* Case Management
– is a service to help clients “manage” comprehensive
care including, medical, social and financial concerns. Case managers
help clients connect with a variety of services and should be contacted
when a client is experiencing problems paying bills, or caring for needs
outside the purview of a CareTeam.
