Medical Case Management

RAIN's Medical Case Management program provides holistic care supporting spiritual, physical, mental, and social wellness. Social Workers and Registered Nurses work together to assist in meeting client needs.

RAIN's program is unique, a "Nurse First Triage" to assist clients in accessing the most appropriate healthcare and gaining greater understanding of your own plan of care. Through Medical Case Management RAIN sees to assist clients with health education, medication compliance, housing concerns, and other issues which ultimately give increased quality of life and greater independence.
